La Rehab makes the referral process easy for you and your patients. We strive to keep you informed and report patient outcomes back to you. Prescription pads, product literature, company brochures and in-services are available upon request.

Sending a Referral to La Rehab
Fax the prescription to La Rehab at 504-468-6109, complete with diagnosis codes and supporting clinical notes.

1. La Rehab will contact the patient to schedule an evaluation if necessary.
2. La Rehab will evaluate your patient and verify their insurance benefits.
3. Our certified Orthotists and prosthetists will fabricate and/or fit the prescribed device to your patient.
4. If necessary, we will schedule a follow-up visit, and then we will report outcomes back to the physician.

It is important to provide an accurate ICD-10 code on all prescriptions and medical record notes for compliance with insurance guidelines. Please include Cause, condition, Rx and notes that support the codes you select.
